- Organize essential documents using a simple, color-coded storage system that allows important information to be located quickly. (A secure digital trusted vault system will be available soon.)
- Create emergency preparedness plans for personal or natural disasters—before a crisis occurs.
- Review existing personal plans to identify and address potential gaps.
- Develop a customized Home Operation Manual, a comprehensive reference guide detailing vital information about the home, belongings, and repair plans.
Through thoughtful planning and expert guidance, families can minimize conflict, recover more quickly from unexpected events, and reduce chaos during times of crisis.
Safe Landing Care Package
Transitions to a new living environment can be emotional and overwhelming. The Safe Landing Care Package ensures these changes are handled with compassion, structure, and expert support.
Our Aging Life Care Advisors™/Care Managers—master’s-level professionals trained in psychosocial motivation and active listening—provide concierge-style services delivered with kindness and meticulous attention to detail. We treat every client like family, with a focus on maintaining dignity and well-being.
The Safe Landing Care Package includes:
- A pre-move meeting (when possible) for client assessment
- A personalized Care Plan outlining the frequency of visits and calls
- Ongoing one-on-one assessments coordinated with the client, family, and community professionals
- Personal advocacy and professional assistance
- Regular updates to family members and caregivers after each visit
Additional services may include:
- Specialized interventions for dementia, anxiety, and depression
- Scheduling follow-up appointments and arranging transportation
- Coordinating food and supply deliveries
- Assistance with errands and home organization
- Coordination of services such as activity plans, food delivery, and private-duty aides
