Springpoint at Home is excited to announce that we continue to grow our Aging Life Care Advisor™/Care Management team.

Kim Bellanfante, BS, MBA

Kim brings over 20 years of experience across social services, healthcare, education, and community-based care. Her professional journey has included roles as a Care Coordinator, Case Manager, Child Protective Specialist with New York City’s Administration for Children’s Services, Early Childhood Education Teacher, Customer Service Manager, and Microsoft Word, PowerPoint, and Excel Trainer for survivors of domestic violence.

One of the most formative chapters of her career was working in an SRO that housed individuals living with serious mental illness, HIV and histories of substance use. She began as a Case Manager and was later promoted to Clinical Supervisor, where she led and supported an interdisciplinary team serving a highly vulnerable, dual-diagnosed population. That experience deeply shaped Kim’s leadership style, clinical lens, and unwavering belief in dignity-centered care. Across every role, the common thread has been advocacy—helping people navigate complex systems, reclaim their voice, and move forward with confidence.

Kim is a mother and a natural caregiver at heart. Connection, integrity, and meaningful relationships are what she values. Kim brings those values into every interaction. “I believe everyone deserves to feel seen, respected, and supported—especially during life’s most vulnerable transitions.”

Kim was drawn to Aging Life Care because it brings together everything she has spent her career building: advocacy, guidance, emotional intelligence and practical problem solving. In her work, Kim has witnessed how overwhelming aging-related decisions can be for individuals and families. Kim’s goal is to be the steady presence who helps make those moments feel less daunting.

“This work allows me to walk alongside people during pivotal life transitions—offering clarity, reassurance, and compassionate support. As an Aging Life Care Advisor, I’m able to honor each client’s story, values, and independence while helping families navigate care with dignity and confidence. It is meaningful, purpose-driven work, and it feels like a natural extension of who I am.”

Kim decided to join Springpoint at Home because “SAH immediately stood out to me for its integrity, mission, and commitment to truly person-centered care. Emily Bell, BS and MSW

Emily has a bachelor’s and master’s in social work from Rutgers University with a certificate in Aging and Health and is a licensed social worker. She recently became a Certified Dementia Practitioner.

Emily interned and worked at the Center for Modern Aging Princeton in the social services team. In that role, she worked in Princeton’s affordable housing program, helping to run programs like Ping Pong Parkinson’s and Special Connections. In addition, she provided referrals and support to anyone in the community. Emily had worked at Meadow Lakes for 8 years in a variety of roles starting as a waitress and that time at Meadow Lakes sparked her curiosity to work with older adults.

“I was drawn to becoming an Aging Life Care Advisor because of my passion to work with older adults in a role that gives me the flexibility to meet them where they are. This role allows me to help older adults in a variety of ways that I wouldn’t be able to do anywhere else. It has given me the opportunity to become a part of a community that offers educational opportunities and support.”

“I was drawn to joining Springpoint at Home because of my desire to stay within the Springpoint community but still grow in my career. Within this role, I can stay within this community and use my experience of working at Meadow Lakes to help our clients. I knew that joining this team would give me the support and opportunities to learn from experienced Aging Life Care advisors™ who come from diverse backgrounds.”
