Frequently Asked Questions:
In-Home Assessment

What is an in-home assessment?

An in-home assessment is a personalized evaluation that helps our team understand your loved one’s health, daily routines, and overall care needs. Whether you’re exploring home care or care management, this visit allows us to recommend the right level of support to ensure safety, comfort, and independence at home.

What is included in a Home Care Assessment?

Our complimentary Home Care Assessment is conducted by a registered nurse and focuses on your loved one’s medical history, physical abilities, and daily living skills such as bathing, dressing, and meal preparation. The nurse also reviews medications and cognitive function to ensure care services are both appropriate and effective.

What is a Care Management Assessment?

A Care Management Assessment is a more detailed evaluation completed by our licensed social workers and gerontologists. It follows eight key areas defined by the Aging Life Care Association – health, disability, finances, housing, family, local resources, advocacy, legal, and crisis intervention – to create a personalized plan that supports safety, happiness, and long-term independence.

How does an in-home assessment help families?

The in-home assessment provides a clear picture of a loved one’s current situation and care needs. It helps families make informed decisions, plan for future support, and access resources that improve quality of life. It also ensures that any services provided are tailored and truly beneficial.

Is there a cost for the in-home assessment?

No. Springpoint at Home offers the Home Care Assessment at no charge when it’s the first step in arranging home care services. Our goal is to ensure every family receives the right guidance and personalized care plan without any obligation or upfront cost.
