Springpoint at Home provides two types of in-home assessments based on your goals. The complimentary Home Care Assessment is completed by a registered nurse and focuses on immediate needs such as daily abilities, medical history and safety concerns. The Care Management Assessment is a separate, in-depth evaluation conducted by our experienced Aging Life Care Managers® and is meant for families who want long-term planning guidance. Together, these assessments offer clarity, direction and a better understanding of the right support for your loved one.

The Care Management Assessment explores eight areas defined by the Aging Life Care Association, including finances, legal considerations, advocacy, crisis support and more. This in-home care assessment results in a personalized care plan that promotes safety, independence and quality of life. Families may also continue working with our care management team to implement the plan.

An in-home assessment is a personalized evaluation that helps our team understand your loved one’s health, daily routines, and overall care needs. Whether you’re exploring home care or care management, this visit allows us to recommend the right level of support to ensure safety, comfort, and independence at home.
Our complimentary Home Care Assessment is conducted by a registered nurse and focuses on your loved one’s medical history, physical abilities, and daily living skills such as bathing, dressing, and meal preparation. The nurse also reviews medications and cognitive function to ensure care services are both appropriate and effective.
A Care Management Assessment is a more detailed evaluation completed by our licensed social workers and gerontologists. It follows eight key areas defined by the Aging Life Care Association – health, disability, finances, housing, family, local resources, advocacy, legal, and crisis intervention – to create a personalized plan that supports safety, happiness, and long-term independence.
