Springpoint at Home offers personal care assistance that is provided by certified home health aides. The certified home health aides are supervised by a registered nurse who will assess your care needs, the safety of your home and develop a personalized care plan. A registered nurse will also perform periodic health assessments and care plan updates and assist in managing access to services and medical benefits.

Certified Home Health Aides

Available on an hourly or 24-hour live-in basis to assist with:

• Supervision
• Companionship and socializing
• Personal hygiene
• Medication reminders
• Meal preparation
• Light housekeeping and laundry
• Motivation and support
• Shopping
• Therapeutic exercise assistance
• Transportation to medical visits

Alzheimer’s / Dementia Care

• Minimize behavioral symptoms
• Encourage engagement
• Support family

Standard Nursing Services

• Provide a clinical evaluation
• Conduct a home safety assessment
• Create a personalized care plan in consult with client and family
• Supervise certified home health aides
• Speak with a designated family member every 30 days
• Conduct a home visit every 60 days
